Anastomosis of the subclavian artery to the distal aorta for management of coarctation of the aorta.

چکیده
T HE CLASSIC EXPERIMENTAL AND CLINical work of Gross and Hufnagel’ on coarctation of the aorta demonstrated the feasibility of direct end-to-end anastomosis of the proximal to the distal aorta for management of the usual type of coarctation in the adult. They pointed out that the anatomic configuration of thecoarctation could be such that considerable mobilization of the aorta would be necessary to re-establish aortic continuity if a long segment of coarctation had to be resected. The problem of bridging very long defects was solved in 19492.3 by the introduction of homologous aortic grafts. An improved technique for the sterilization and preservation of such homologous arterial grafts was introduced by Meeker and Gross4 in 1951.

Selection strategy and technical variations of extraanatomic bypass in surgical management of complex and recurrent aortic coarctation and hypoplastic aortic arch
Method In 1989-2012, 60 patients (mean age 29 ± 6.7years) with complex aortic coarctation (n = 33), recurrent coarctation (n = 27; anastomosis pseudoaneurysm in 10), underwent correction using extraanatomic bypass, either or without extracorporeal circulation. The decision to use extracorporeal circulation was based on the anatomical location of the coarctation, length of hypoplasia and history...
متن کاملHypothermic circulatory arrest through a left thoracotomy in a 12-year-old child with aortic coarctation.
Surgical correction of adult complex aortic coarctation using hypothermic circulatory arrest often requires central cannulation to secure cerebral perfusion. It is not easy to place the cannula in the ascending aorta, however, especially in children undergoing surgery through a left thoracotomy. In a 12-year-old male with hypoplastic distal aortic arch, we placed an arterial cannula in the asce...
متن کاملReverse subclavian flap repair and maintenance of antegrade blood flow within the left subclavian artery in neonates with aortic coarctation and distal arch hypoplasia.
Surgical palliation for aortic coarctation with aortic arch hypoplasia in neonates and infants has been used in the clinic as the most beneficial treatment for this disorder. This technique allows the correction of aortic coarctation by the use of "extended" anastomosis without cardiopulmonary bypass, which expands the hypoplastic distal aortic arch via the use of a reverse subclavian flap repa...
متن کاملAnatomical Study of Subclavian Artery and Its Branches
Background: The branches of right and left subclavian arteries are important in conducting of blood to spinal cord, posterior cranial fossa, base of the neck, thyroid gland, wall of the thorax and abdomen. The routine use of internal thoracic artery as a conduit in coronary artery bypass grafting surgery requires appreciation of the anatomical variations of these vessels. So far, there is not a...
متن کاملAcquired Infrarenal Abdominal Aortic Coarctation: Treatment with Percutaneous Self Expandable Stent
Abdominal aortic coarctation is an extremely rare vascular pathology. Its etiology can be congenital or acquired. Here we present a case of acquired infrarenal abdominal coarctation in a 66-year-old woman who complained of low back and legs pain. She had no signs of resting lower limb ischemia, with diminished distal pulses and normal blood pressure in upper and lower extremities. Magnetic reso...
